  • 看護系大学における養護教諭養成課程開講の現状

    日本における養護教諭養成教育を行っている看護系大学78校の開講状況について明らかにすることを目的として,各養成校のホームページを調査した。近年,看護系大学は看護師国家試験受験資格のみ取得できる養成校が多く,資格取得のためにも養護教諭養成教育のニーズは高いと考えられた。しかし,看護学教育がメインのカリキュラムのなかで養成されているために,開講されている養護に関する専門科目が少ないこと,学校現場における実習の機会も医療現場での実習に比べて少ないことがわかった。このことは,学生自身の教育職であるという自覚の低さや養護教諭としての質の担保の難しさに繋がり,看護系大学の養護教諭養成教育全体における課題であると考えられた。(著者抄録)
  • 保健師養成機関における「公衆衛生看護の倫理」教育の実態

    目的 公衆衛生看護の倫理に関するモデルカリキュラム・教育方法・教材開発のために、全国の保健師養成機関における倫理教育に関する実態を把握することを目的とした。方法 全国の保健師養成機関(専修学校(1年課程の保健師養成所、4年課程の保健看護統合カリキュラム校)、短大専攻科、大学)229校に質問紙を送付し、公衆衛生看護教育を担当する教員に回答を求めた。質問紙の内容は、回答者および所属機関の属性や保健師資格教育の形態のほか、公衆衛生看護の倫理の独立・関連科目の有無と導入予定、公衆衛生看護以外の倫理科目、公衆衛生看護の倫理を学ぶことの重要性や望ましい対象など、担当できる教員の有無やその研修の必要性、教育にあたって必要な資源、公衆衛生看護の倫理として扱う内容などを尋ねた。回答は変数ごとの記述統計量を算出するとともに、自由記載の内容分析を行った。結果 全国の保健師養成機関に質問紙を送付し、89校(回収率38.9%)から回答を得た。保健師養成機関の内訳は大学78.7%、短大専攻科4.5%、専修学校9%であった。公衆衛生看護の倫理の独立科目はなく、9割近くは導入予定もなかった。42.7%が科目の一部で公衆衛生看護倫理を扱っていた。公衆衛生看護倫理を学ぶ重要性については「非常に重要・ある程度重要」を合わせて9割であった。58.4%が保健師教育において公衆衛生看護の倫理に関する授業を必須化する必要があると回答したが、倫理教育を担当する教員については4割以上が「いない」と回答した。教員の研修は8割以上が必要と答え、必要な研修形態は「専門職団体や学会などによる学外研修」が8割と最も多かった。必ず行う必要があると思われる公衆衛生看護の倫理教育の内容の上位は「公衆衛生看護実践者としての職業倫理」、「健康と基本的人権」、「個人情報とその保護」、「公衆衛生看護における倫理」、「公衆衛生看護における倫理的自己決定」であった。結論 公衆衛生看護倫理教育はその必要性は高く認識されているものの、実施率は低かった。モデルカリキュラム、教材、教授できる教員が不足していること、教授が必要とされる公衆衛生看護の倫理の教育内容が体系化されていない現状が明らかになった。公衆衛生看護倫理の定義の合意形成と、モデルカリキュラムと教育方法、教材の開発、教員の養成が急務であると考えられた。(著者抄録)
  • 初任期養護教諭が成長する経験とその要因

    目的:本研究の目的は、初任期養護教諭が成長する経験とその要因について明らかにすることである。方法:経験4〜6年目の現職養護教諭6人を対象に、2014年8月〜2015年3月に半構成的インタビューを行った。内容は、初任から現在までを振り返って、自身が養護教諭として成長したと感じた出来事、エピソードについて自由に語ってもらった。結果:6人の養護教諭が語った質的データを分析した結果、初任期養護教諭が成長する経験は7カテゴリーとそれらに基づく5つのコアカテゴリーが生成され、さらに成長する経験に関連する6要因を抽出した。初任期養護教諭は、新人養護教諭として【何をすればよいのかわからない漠然とした不安】を抱いているが、日々の実践において、専門職としての力量形成、児童生徒への適切な理解と対応といった能力的成長とともに、学校組織における関係性構築、仕事に対する理解とやりがい、養護教諭としてのアイデンティティーの獲得という精神的成長につながる経験をしていた。また、その経験をもたらす要因は、【経験を積み重ねること】【内省すること】といった個人要因と【他の養護教諭の存在】【異動などによる役割の変化】【理解がある管理職や教職員の存在】【連携・連絡体制が良好な学校組織】といった環境要因がかかわっていた。考察:養護教諭として成長していくためには、実践を重ねながら内省し、学び続けること、学校という組織の一員として関係性を構築するための努力をするという姿勢が求められる一方で、養護教諭同士のつながりや校内組織における学校保健活動と養護教諭の職務への理解といった組織のあり方の課題も示唆された。(著者抄録)

    Objective: To determine the characteristics of nursing research considered by the IRBs and how to improve IRBs reviewing nursing research in Japan in comparison with IRBs in the USA with respect to types of reviews, the IRB reviewing systems.

    Methods: We classified nursing research based on the research characteristics through a literature review, and obtained information about the types of reviews and review systems of IRBs from two universities with nursing departments in the USA.

    Results: (1) Nursing research is classified into four types (research, quality improvement, evidence-based practice, and program evaluation). (2) Depending on the anticipated risks of research subjects, protocol reviews are classified as follows: exemption, expedited, and full board. (3) In the USA, research proposals are generally reviewed by a full IRB, but there is a sub-committee for expedited review. The reviewing system is very well-organized with sufficient staffing, which allows distribution of staff to various sections, including a consulting section for inquiries of researchers. (4) The objectives of nursing research are largely associated with improvement in nursing care, evaluation of programs, and evidence-based practice, and these types of research are categorized as exempted or expedited reviews.

    Conclusion: There are significant needs for improvement in the IRB system in Japan regarding reviewing procedures, which should be modified depending on the methods of nursing research, subjects, and content. In addition, there is a need for staff development so staff can provide advice or consultation with researchers who want to apply to the IRB.

  • Developing a scale to measure "attachment to the local community" in late middle aged individuals

    Objectives This study was conducted to develop a scale for measuring "attachment to the local community" for its use in health services. The scale is also intended to nurture new social relationships in late middle-aged individuals.

    Methods Thirty items were initially planned to be included in the scale to measure "attachment to the local community", according to a previous study that identified the concept. The study subjects were late middle-aged residents of City B in Prefecture A, located in Tokyo suburbs. From the basic resident register data, 1,000 individuals (local residents in the 50-69 year age group) were selected by a multi-stage random sampling technique, on the basis of their residential area, age, and sex (while maintaining the male to female ratio). An unsigned self-administered questionnaire was distributed to the subjects, and the responses were collected by postal mail. The collected data was analyzed using psychometric study of scale.

    Results Valid responses were obtained from 583 subjects, and the response rate was 58.3%. In an item analysis, none of the items were rejected. In a subsequent factor analysis, 7 items were eliminated. These items included 2 items with a factor loading of <0.40, 3 items loading on multiple factors and showing a factor loading of ≥0.40, and 2 items with a low factor correlation (0.04-0.16). These items included factors that related to only these 2 items. Consequently, 23 items in the following 4-factor structure were selected as the scale items: "Source of vitality to live life," "Intention to cherish ties with people," "Place where one can be oneself," and "Pride of being a resident." Cronbach's coefficient α for the entire scale of "attachment to the local community" was 0.95, demonstrating internal consistency. We then examined the correlation with an existing scale to measure social support; the results revealed a statistically significant correlation and confirmed criterion-related validity (P<0.001). In addition, the fit indices in a covariance structure analysis showed adequate values.

    Conclusions The developed scale was considered reliable and appropriate for measuring "attachment to the local community."

  • Concept Analysis of "Attachment to the Local Community" for Public Health Nursing

    Purpose: This paper presents the concept analyses of 'attachment to the local community' including: individuals' inner awareness; relationships between individuals and relationships with their local community. The rationale for adopting the concept for public health nursing practice is identified and useful public nursing activities are suggested.
    Methodology: We interviewed nine participants selected for their social activities in their local community. Concept analysis yielded: concept characteristics, factors affecting the formation of the concept and the outcome expected from the formation of the concept, which were classified into categories. Focusing on relationships between the categories, we examined the overall structure of the concept at three levels: personal, inter-personal and community (ranging from the neighborhood to municipality).
    Results and discussion: The findings indicated that the categories interact at each level to bring about inner and external changes among individuals. The concept reflected a process structure. Progress in the formation of 'attachment to the local community' stimulates community-oriented behavior, thereby generating better and greater outcomes than expected at the formation stage. Thus, 'attachment to the local community' may be defined as a supportive mind-set and forward-looking intention towards their local community that is formed by sharing experiences with others in their day-to-day living situation and which changes as they interact with social circumstances.
    Conclusion: Our study implies that the concept is useful for the practice of public health nursing aimed at both improving individuals' QOL and building local capacity.

  • ヘルスプロモーション実習における実習の場による学びの特徴

    2011年度から本学の地域看護実習は、2週間を1ヶ所の保健所・保健センター等で実習するAコース、前半1週間を自治体の敬老館または児童館、後半1週間を保健所・保健センター等で実習するBコースと2通りの実習方法を併用するヘルスプロモーション実習を導入した。学生がそれぞれの実習方法でどのような学びを得ているのか、地域看護実習の目標の到達状況の評価の比較を行い、より効果的な実習内容・方法について検討した。「実習目標到達度の自己評価」では、21項目中2項目にAコースとBコースに有意差が生じていた。「実習における学び」の記述では、対象の捉え方や保健師活動など着目した点に両コースの特徴が表れていたが、実習目標すべてについて考察されており、目標に到達していることが確認できた。今後は、保健所・保健センターにおける実習に加えて、住民と直接接触できる、より地域の中に入り込んだ実習が有効な学びにつながることが示唆された。(著者抄録)

    The objective of this article is to provide an overview of the St. Luke's College of Nursing 21st Century COE Program7th International Relay Symposium “Let's Learn about Our Body with Children!“ and to consider the components of peoplecenteredcare by analyzing and evaluating the process by which the symposium was conducted.The objective of the symposium was to provide methods through which five-year-old children can learn about thehuman body and to facilitate the meaningful exchange of opinions with the general public. The symposium was targetedfor five-year-old children and their parents, as well as various specialists working with five-year-olds, such as childcareprofessionals. kindergarten teachers, and nurses and school health nurses.The symposium was planned and carried out in cooperation with the general public. Each event on the program waskept to within 20 minutes, and a picture story show and song and dance were included in the program to ensure thatchildren did not lose their interest.Several things were achieved through the process of working cooperatively with the general public in order to conductthe symposium.   • Characteristics of the Users of Health Consultation Service Offered by a Nursing College

    Purpose: A private nursing college research center, located in Tokyo, established a place to provide citizens with free health consultation with the aim of promoting People-centered care. The purpose of this study was to analyze characteristics of the service users, patterns of service use, and the reasons for visiting in order to plan for the future and design an ideal health counseling service. Method: The consulting records for the period from April 2005 to March 2006 were reviewed. Categories that had been identified during an earlier study were used to classify the data. Results: A total of 577 people (402 women; 151 men), 67% of whom were over 50, visited the facility to receive the service; 70 (12.1%) used the service more than once. The purposes of their visits are as follows: "to understand his/ her own health condition" (458), "to consult about symptom for which diagnosis has not been made" (286), and "to consult about diagnosed disease" (214). Other topics of consultation, accounting for 25% of the 1,247 reported health concerns.included: "daily health maintenance," "results of medical examination," "how to receive medical service," "body mechanisms," "how to communicate with medical professionals," "how to obtain medical information," "how to communicate with one's family member/acquaintance with an illness," "everyday interpersonal issues," "one's own life and future," "activities of the medical care facility," and "greetings/what one has been doing lately". Discussion: Citizens used the health consultation service at the nursing college as "an opportunity to get a grasp of one's health condition" or "an opportunity to seek advice about a symptom for which no diagnosis has been made." Their visits also served as "an opportunity to seek advice about one's anxiety or concern about a diagnosed disease." We conclude that the current services seem to meet important needs for health counseling among users.

    The objective of this article is to provide an overview of the St. Luke's College of Nursing 21st Century COE Program 7th International Relay Symposium "Let's Learn about Our Body with Children!" and to consider the components of people-centered care by analyzing and evaluating the process by which the symposium was conducted. The objective of the symposium was to provide methods through which five-year-old children can learn about the human body and to facilitate the meaningful exchange of opinions with the general public. The symposium was targeted for five-year-old children and their parents, as well as various specialists working with five-year-olds, such as childcare professionals, kindergarten teachers, and nurses and school health nurses. The symposium was planned and carried out in cooperation with the general public. Each event on the program was kept to within 20 minutes, and a picture story show and song and dance were included in the program to ensure that children did not lose their interest. Several things were achieved through the process of working cooperatively with the general public in order to conduct the symposium. The elements of people-centered care acquired through past COE activities were confirmed. The staff members experienced the process of "drawing out and exposing (ascertaining them from each other) hidden needs within the community, and shifting activities towards cooperative efforts with specialists and further developing those activities". In addition, a new important element in such activities, "ascertaining things from each other", was uncovered. The symposium also allowed us to shift the focus of cooperation with the general public, something that we had initially been searching for ways of achieving effectively, towards vibrant and energetic cooperation with the community. Moreover, it was confirmed that the process of cooperation with the general public has continued to develop, moving towards symposiums whereby the community and specialistswork together in search of further progress in their activities. Several future issues were re-identified: the status of further cooperation with the community, and concrete methods for evaluating methods by which five-year-olds can learn about the body.

    This research is on the assumption that knowledge of the body is necessary for people to take initiative in their own healthcare. Prior research indicates that age 5 is the best age to start systematic education about the body. We have developed an educational program about the digestive system. This program was presented to 69 5-year-olds in two nursery schools and two classes of a kindergarten. Teaching aids include a panel theater ('kamishibai'), picture book and a T-shirt depicting the digestive system. Nurse teachers presented the story "How an apple becomes a BM" using the panel theater. Then, the nurse teachers reviewed what they had presented, while manipulating the special T-shirt. From the T-shirt teachers could remove the parts, open the organs and show the stuffed fabric representing apples at various phases in their digestion. Lastly, each preschooler was given a picture book to take home. Forty-five mothers and five nurse teachers responded to an evaluation questionnaire and indicated that this program was an incentive to learn about the body; that they expected a series of the programs; and that after the preschoolers returned home, they talked with their mothers and other family members about the digestive system using words they learnt. From our work we recognize that partnership with parents and nurse teachers is critical for continued program development. A cohort study is needed in order to evaluate the contribution of our program to understanding of the body.
