  • What People Gain from Dog Ownership.

     For the purpose of recording what humans gain from dog ownership, I conducted semi-structuredinterviews and pace count surveys with nine male and female dog owners. The results thereof were thatowning a dog meant “gaining a partner for healthy living,” a condition composed of seven categories:“reconstructing life with the dog,”“ nurturing responsibility toward others,”“ gaining a companion in the dogfor healthy activity,”“ being soothed by the dog resulting in mental calmness,”“ finding the dog irreplaceable,”“becoming closer as a family through the dog,” and“ nurturing interest in others through the dog.” It is not easy to transfer the direct advantages of dog ownership to all residents, but the intermediationof the dog allows for going out into the community while walking the dog, interacting with others, actingwith mutual consideration, and taking part in local activities, thus joining in a network and making benefitsto the community possible. Thus, further research is called for regarding dog ownership and human healthpromotion, increased interest in social interaction, and participation in local activities.

  • 向老期世代における"地域への愛着"測定尺度の開発

    目的 向老期世代における新たな社会関係の醸成と保健事業での活用を目指し、"地域への愛着"を測定するための尺度を開発することを目的とした。方法 "地域への愛着"の概念を明らかにした先行研究に基づき合計30項目を"地域への愛着"の尺度案とした。対象は東京近郊に位置するA県B市の住民とし住民基本台帳データより、50〜69歳の地域住民から居住エリア・年代・男女比に基づき1,000人を多段階無作為抽出し、無記名自記式質問用紙を郵送にて配布・回収した。収集されたデータを用いて尺度の計量心理学的検討を行った。結果 583人から有効回答が得られた(有効回答率58.3%)。項目分析では項目の削除はなかった。次いで因子分析を行い、因子負荷量が0.40未満の2項目、複数の因子にまたがって0.40以上であった3項目、因子間相関が0.04〜0.16と低くかつ項目数が2項目と少なかった因子に含まれる2項目の計7項目を削除し4因子構造23項目を採用し尺度項目とした。各因子は"生きるための活力の源"、"人とのつながりを大切にする思い"、"自分らしくいられるところ"、"住民であることの誇り"と命名した。"地域への愛着"尺度全体のCronbachのα係数はα=0.95であり内的整合性が確認された。既存のソーシャル・サポートを測定する尺度と相関をみたところ統計学的に有意な相関があり(P<0.001)基準関連妥当性も確認された。また、共分散構造分析による適合度指標も十分な値を示した。結論 開発した尺度は"地域への愛着"を測定する尺度として信頼性・妥当性を有すると考えられた。(著者抄録)

  • 保健師養成機関における「公衆衛生看護の倫理」教育の実態

    目的 公衆衛生看護の倫理に関するモデルカリキュラム・教育方法・教材開発のために、全国の保健師養成機関における倫理教育に関する実態を把握することを目的とした。方法 全国の保健師養成機関(専修学校(1年課程の保健師養成所、4年課程の保健看護統合カリキュラム校)、短大専攻科、大学)229校に質問紙を送付し、公衆衛生看護教育を担当する教員に回答を求めた。質問紙の内容は、回答者および所属機関の属性や保健師資格教育の形態のほか、公衆衛生看護の倫理の独立・関連科目の有無と導入予定、公衆衛生看護以外の倫理科目、公衆衛生看護の倫理を学ぶことの重要性や望ましい対象など、担当できる教員の有無やその研修の必要性、教育にあたって必要な資源、公衆衛生看護の倫理として扱う内容などを尋ねた。回答は変数ごとの記述統計量を算出するとともに、自由記載の内容分析を行った。結果 全国の保健師養成機関に質問紙を送付し、89校(回収率38.9%)から回答を得た。保健師養成機関の内訳は大学78.7%、短大専攻科4.5%、専修学校9%であった。公衆衛生看護の倫理の独立科目はなく、9割近くは導入予定もなかった。42.7%が科目の一部で公衆衛生看護倫理を扱っていた。公衆衛生看護倫理を学ぶ重要性については「非常に重要・ある程度重要」を合わせて9割であった。58.4%が保健師教育において公衆衛生看護の倫理に関する授業を必須化する必要があると回答したが、倫理教育を担当する教員については4割以上が「いない」と回答した。教員の研修は8割以上が必要と答え、必要な研修形態は「専門職団体や学会などによる学外研修」が8割と最も多かった。必ず行う必要があると思われる公衆衛生看護の倫理教育の内容の上位は「公衆衛生看護実践者としての職業倫理」、「健康と基本的人権」、「個人情報とその保護」、「公衆衛生看護における倫理」、「公衆衛生看護における倫理的自己決定」であった。結論 公衆衛生看護倫理教育はその必要性は高く認識されているものの、実施率は低かった。モデルカリキュラム、教材、教授できる教員が不足していること、教授が必要とされる公衆衛生看護の倫理の教育内容が体系化されていない現状が明らかになった。公衆衛生看護倫理の定義の合意形成と、モデルカリキュラムと教育方法、教材の開発、教員の養成が急務であると考えられた。(著者抄録)

    In 2008,the Student Affiirs Division of St. Luke's College of Nursing launched an initiativefor acquiring appropriate communication skills and basic manners so that students caneffectively fulfill this institution's educational objectives and benefit from awell-rounded campus life. It is three years since students started the activitiesunder the slogan of “Establishing a suitable learning environment“. In 2010,they continued to carry on these activities, making a valuation of what they did forthese three years. The actual activities in this year were: promotion of exchangegreetings, implementation of public relations at the school festival,campaign for collecting plastic bottle caps. The feedback on the aboveactivities from their peers and teachers showed the expansion of good manner awareness.However, some bad manners were still pointed out, and the activities to promotegood manners needed to be continued. In order to further promote and appreciatethese activities, faculty staff could show support for such studentinitiatives, cooperating closely with other relating committee members.
  • Characteristics of Learning Based on Different Facilities in Health Promotion Practice.

     Starting from FY2011, the public health nursing practice has launched two kinds of health promotioneducational practicums for comparison of effectiveness. The first was “A” course, a two week practicaltraining at a public health center and the second was “B” course composed of one week of practical trainingat a municipal senior citizen center or children’s welfare facility and then one week at a public health center. We reviewed more effective and practical methods of teaching and contents by inspecting what studentswere learning in their practical training and also evaluating how their goals for public health nursingpractices were achieved. There were significant differences in two out of 21 items between “A” and “B” courses regarding “Selfevaluationfor Goals of Practical Training”. Written reports for“ Learning in Practical Training” pointed outhow students from both courses coped with the situations and that they were able to closely observe hownurses ‘on the job’ practiced public health nursing.   • Learning Effect of a Practical Exercise Conducting a Community Assessment

    Starting from FY 2009 to FY 2013, the community health nursing department has launched a practical exercise conducting a community assessment as an elective course open to seniors. This paper describes the model course, workbook, previous theme of community assessment, and discusses the learning effect and application for the new curriculum.The aim of this course is to develop students’ practical capabilities for community assessment based on the characteristic of the target population. The learning effects of the students were“ getting vibrations from the community and gaining the opportunity to join the community,”“ pursuing what health means for the people who work in the organization,” “understanding the linkages among home visit nursing, legal system, and administration.” The students had clear objectives and high commitment to the community, and through the process of their practice, they developed a positive attitude as a member of the health care team.Implementing the new curriculum takes us in the direction of establishing links with experience in community assessment and how that assessment could be applied to community health.   • Ethnography of administrative organizations related to the ethical practice of public health nurses in Japan

    The purpose of this study was to describe the relationships between the culture of Japanese administrative organizations and the ethical practice of public health nurses (PHNs). We collected data by conducting interviews, observation of participants, and examination of existing materials, and then the data were analyzed by content analysis. The results showed the culture of the administrative organizations was “accomplishing projects faithfully based on the procedures of their organizations or laws/systems,” while the culture of the PHNs was “promoting health and well-being of the population using their professional knowledge and skills.” Cognitive gaps between the administrators and PHNs occurred with interaction of these two cultures, particularly with regards the ethical practice of the PHNs’. PHNs and administrators must therefore recognize their different cultures, and develop a culture in which they can work together as partners.

  • Development and Evaluation of an Educational Ladder and Relevant Programs for Ethics Education in Community Health Nursing Practice

    The purposes of this study were to develop an educational ladder and the relevant programs for ethics education to promote effective practice of public health nurses (PHNs) and home visiting nurses (HVNs) in Japan. We conducted the interviews and questionnaire surveys with PHNs and HVNs to clarify : the contents and frequencies of encountering ethical issues in their practices, the components of moral behavior, experiences with ethics education, experiences influencing nurses' decisions in practice and workplace environment related to ethical practices; then the relationships between each variable and demographic data including length of work experience and position were analyzed. Based on these results, we developed a tentative educational ladder and targeted programs for ethics education and then conducted and evaluated the trial programs for PHNs and HVNs. Finally, we revised the ethics educational ladder and the relevant programs for ethics education for PHNs and HVNs.
  • Development of a voluntary health promotion program for younger elderly women based on exchanges in the community

    This paper is about a pilot program entitled "Let's think of ourselves 10 years from now (future blueprint: future tree)" which targeted middle-aged and older women in a new residential area. We conducted a year long continuous evaluation of the program which applied several methods including questionnaires and interviews at home. We found positive effects in participants on : 1) changes in their recognition about their relationships with neighbors ; 2) maintenance and improvement of QOL ; and 3) proactive health improvement activities in their community. Thus we confirmed that the program is beneficial in improving health and regional development at the same time.
