Objectives The amount of research relating to evaluation of intervention programs with regard to sense of coherence, a concept of salutogenic stressor coping capacity, is increasing in Japan. However, a nationally representative score of the Japanese version of the sense of coherence scale has not yet been reported. The aim of this study was to describe the 13-item, 7-point Japanese sense of coherence scale (SOC-13), with scoring by sex and age group, and to examine the relationships between the score and locality and city size.
Methods Four thousand Japanese men and women were selected by stratified random sampling, and a cross-sectional, self-administered questionnaire survey was conducted using the placement method. Responses were obtained from 2,067 individuals (response rate: 51.7%). This study analyzed 956 men and 1,107 women (mean age [standard deviation]: 50.0 [14.3]).
Results The mean score on the SOC-13 was 59.0 (12.2) in all participants, 59.1 (11.8) in men, and 58.9 (12.5) in women. No significant difference was found between men and women (P=0.784). ANOVA and multiple comparison for age difference showed a clear relationship (P<0.05) between higher age and a higher SOC-13 score. Moreover, the results of main and interaction effects in ANCOVA, with independent variables for locality (11 segments) and city size (four segments), were not significant when controlled for age.
Conclusion A nationally representative score for the Japanese SOC-13 was acquired. In future research, application of the SOC-13 in Japan for clinical studies is anticipated based on the nationally representative score.

     本研究は、看護師養成機関における性同一性障害(gender identity disorder : 以下GID とする)学生の演習・実習指導などについて、受け入れ経験者から得た具体例を検討した。対象は、身体的性は女性であるが自己認識は男性であるFTM(Female to Male Transsexual)学生の受け入れ経験のある教員3名である。

  • Trial Introduction of e-learning as a Tool for Reviewing Nursing Skills.
     10(1) 54-60 Jun. 2006
The use of e-learning as a convenient educational method has been pointed out as being effective for thecontinuing education of nurses who often their irregular working hours. As a preliminary step in exploringthe possibilities of utilizing e-learning in the continuing education of nurses, our school introduced elearningtools for undergraduates studying the fundamentals of nursing from November 2003. The intendedaudience of the e-learning tools was sophomore and freshman students of the nursing college. The studentswere provided with e-learning contents on nursing skills as supplementary learning aids to theirlectures and lab classes. From the results of a web-based self-survey regarding the e-learning tools, themajority of students replied positively with statements such as “the e-learning materials were usefulfor studying“, “I want to continue using e-learning tools in the future“, and “the e-learning tools shouldbe more interactive“. Only a few students agreed with the statement that “credits should be awardedbased only on e-learning materials“. This trial suggests that e-learning is an effective resource forlearning nursing skills. However it is necessar

In order to establish the causal model structure for e-learning needs, we investigated the needs ofnurses for e-learning by a web and a mailing method, and subsequently compared the results by employingcovariance structural analysis. We investigated the e-learning method by placing a series of questionnaires on our college websitefrom May to October2003(web group). For the mailing group, we sent the same questionnaire to a random stratified sample of nurses in Japan, from February to April 2003. Included in the survey were public health nurses, midwives, and nurses working in health, medical, welfare and educational institutions, and companies. We excluded nurses working in clinics, public health centers, and unspecifiedinstitutions. We assumed a response rate of 30% and performed stratification random sampling with a 0.4% rate of extraction. This left us with 810 institutions with 3,466 nurses from the total number of 1,103,760 nurses, for the mailing. The results were as follows: 1) We obtained 460 responses from the web group, and 1,270 responses from the mailing group (36.6% of valid response rates from the mailing group). • Current status of e-learning in Japanese Universities and Colleges -Analysis of web sites and literature review
    (30) 81-93 Mar. 2004
E-Learning is an advanced online technology method of continuing education in Japanese nursingcolleges. E-Learning allows students to learn because they can adjust their work-time schedules andit removes physical access barriers to educational facilities. E-Learning is a two-way system betweenteachers and students through the Internet. This article is a report showing the present status of E-Learning at 33 universities and colleges inJapan identified as using this technology. Web site search results and a review of journal articlescontaining descriptions of E-Learning programs were analyzed using 10 factors: course name, coursedescription, course philosophy, credit, cost, admission requirement, entrance examination, class-sizelimit, E-Learning educational content, and computer system tools.Among the 15 nursing college programs analyzed, only one nursing college introduced E-Learninginto their master's course. In the 18 liberal arts university and college programs analyzed, we foundmany E-Learning course variations have been developed. There was no similarity among the variousuniversity and college programs related to cost, class size or course name, howeve

The utility of e-learning for Japanese nursing education, with its value of faculty-student interaction, quality clinical application and close supervision have been questioned. To examine the utility of introducinge-learning into St. Luke's College of Nursing, a literature review was conducted in order toexplore the basic framework of e-learning. From the review it was evident that e-learning, which is theuse of multimedia technologies and the internet to improve access to learning resources, had two dimensions. These two dimensions that supported e-learning were: distance education, which is learningat a distance outside the classroom and Computer Assisted Instruction (CAl), which is the use oflearning programs on the computer. In addition two directions in e-learning were found; one as thevector of “generalization“ which provides people a wide educational opportunity, and the other as thevector of “individualization“ which promotes active learning. Finally e-learning evaluations were comparedwith traditional lecture type course evaluations in the field of nursing education. No major differenceswere found between academic achievement using e-learning and tr

To examine the possibility of introducing e-learning into graduate schools of nursing in Japan, aweb survey was conducted first in order to explore the basic framework of e-learning.The URL gradschools.com was used as the method for the Web survey. The URL resulted in a listof 17 institutions of higher learning that included 3 colleges and 14 universities, 98 courses. Some institutionsof higher learning limited accreditation to individual courses, while other such institutions accreditedthe whole department. The database of each university and college using: the name of university, URL of the homepage, course category, accreditation status, course credit, course term, cost per credit“ on or off campus, clinical training, the kind of examination, main classroom and study tools, online library, support systemof information technology, and computer literacy requirements.

Ministry of Education, Culture, Sports, Science and Technology:Grants-in-Aid for Scientific Research(基盤研究(B))
    Project Year: 2009 - 2011  Investigator(s):Junko TASHIRO
A web-based learning program which was"reflective log or journal"in order to strengthen the competency of reflective practice, especially for students practicing in remote a clinical field based on a literature review of"Reflective Practice"and hearing from experts in education using reflection in England. Graduatestudents used this e-reflective log reported that they could reflect on their daily practicum by themselves and communicated with their faculty without a time-gap. However, some faculty members stated their comments to students were sent byusing e-mail which was not incorporated to this web-system and therefore theydidnot remain and faculty could not monitor their changes. This web-based reflectivelog system should be developed and become a more comprehensive learning, as wellascommunication, tool for both students and faculty.

  • Developing platform of shared data of infant sleep analyzing system
Ministry of Education, Culture, Sports, Science and Technology:Grants-in-Aid for Scientific Research(基盤研究(B))
    Project Year: 2007 - 2010  Investigator(s):Hiromi ETO
The aim of this study was to develop a home-monitoring system for recording and analyzing infant movement using image processing during nocturnal co-sleep, and to construct platform to network among pediatric sleep researchers. The equipment for the video monitoring system consisted of digital camera, Vari-zoom-lenses, near infrared LED, Image Recording Tool (including a computer to record infant's state), and Video Analysis Viewer for automatic analyses. We examined whether this system was robust for recording and detecting three infant states : quiet sleep, active sleep and awake. The range of interrater agreements of each infant's state was between 0.59-0.77. This newly developed video monitoring system shows good potential as a reliable robust system able to automatically detect infant-sleep wake state.

  • Development and Evaluation of a Decision-making Supporting Prenatal Genetic Nursing Education Program and an e-learning System.
Ministry of Education, Culture, Sports, Science and Technology:Grants-in-Aid for Scientific Research(基盤研究(A))
    Project Year: 2004 - 2007  Investigator(s):Naoko ARIMORI
In study, we developed and evaluated a perinatal genetic nursing education program through an e-learning system. We created this program based on our preceding research, "The Competency of Genetic Nursing Practice". It includes 7 parts, i.e. "Roles in the medical team", "Clarification of the purpose of visits and information collection", "Basics of clinical genetics", "Congenital anomalies and late child-bearing", "The practice and guidelines of prenatal tests", "Decision-making aid", and "Care of women who are having abortions and their families". To assess this program, prior and afterward problems were provided. This program is composed of "Practice part", which is used to confirm the knowledge learnt, and "Document part", which provides references to fit the needs of the attendees. This program is a "decision-making adult learning method (andragogy)". The attendees choose the contents by themselves. In order to attract the attendees' interest, we provided video for practical skill of amniocentesis, and introduced books and websites. "Bulletin board" and "tutor system" were provided to strengthen a sense of community among attendees, and to maintain their learning motivation. Fliers and websites were utilized for recruiting. As a result, 75 attendees registered to this program. For evaluation, (1) outcome evaluation (prior and post examination) and (2) process evaluation (the actual status, questionnaires, and interviews) were conducted. 95% of the attendees passed the acceptance line of 80% achievement. The target group among the attendees also showed increase rates exceeding the acceptance line. In the process evaluation, we found that each learning session was within 30 minutes, and both the "Practice part" and the "Document part" were referred. Support systems such as bulletin board on website obtained high evaluation. Most attendees were satisfied with this program. Through interviews, it was clear that the expectations to meet post-graduate educations as well as e-learning became very high.
  • DEVELOPMENT OF A NURSING MANAGEMENT MINIMUM DATA SET IN JAPAN
Ministry of Education, Culture, Sports, Science and Technology:Grants-in-Aid for Scientific Research(基盤研究(B))
    Project Year: 2004 - 2006  Investigator(s):Toshiko IBE
The purpose of this study is to develop a Japanese version of Nursing Management Minimum Data Set (NMMDS-j), which is composed by a minimally necessary indexes measuring competency required for every nurse manager (NM) facing with management of daily practices.1 Confirmation of the reliability/propriety of NMMDS-jNMMDS-j consists of 6 categories such as, plan, motivation, education, communication, organization and safety, which aims to measure dimensions of nursing management, and 7 questions are comprehended in each category. The framework of NMMDS-j is established by employing traditional management theories, and the trial test using NMMDS-j (β) was held in 2005. After some refinements of NMMDS-j (β) by results of the trial test, a large-scale survey was performed in July-October 2006 in order to investigate reliability/propriety of NMMDS-j (version 1).4678 questionnaires were sent to 579 hospitals throughout Japan, and responses were received from 1762 NMs (response rate 37.7%)). Based on the received data the-reliability/propriety of NMMDS-j (version 1) were confirmed. Results also suggested the background information, including educational background and the years of experience as a manager. Situations of each hospital or wards were considerably related to some competencies of nursing management measured by the NMMDSj.2 Promotion of NMMDS-j.This study revealed some competencies basically needed for NM s. Through discussions with several NMs, NMMDS-j (version 1) was confirmed as a useful self evaluation tool. In addition, it was recommended as an effective educational tool for new NMs to give theoretical suggestions. Moreover, as our study revealed that NMMDS-j (version 1) succeeded to convey practical knowledge into language, we expect that it can be a common language for NMs to share their information. As continuously refining NMMDS-j by taking the social trend into consideration, we can safely conclude that it is worthwhile to promote NMMDS-j among NMs all over Japan.
  • Satellite system for observation of infants with inverted day-night sleeping cycles and their primary caregivers
Ministry of Education, Culture, Sports, Science and Technology:Grants-in-Aid for Scientific Research(基盤研究(B))
    Project Year: 2004 - 2006  Investigator(s):Hiromi ETO
In order to observe the sleep-awake states of infants and identify the characteristics of these states, efforts were devoted to developing software that can objectively collect data and analyze it. This system focused on infant body movement, based on observation and analysis methods using videosomnography. In 2004, the system was developed and the software almost completed. The time-lapse video set/system was changed to a digital camera and computer, reducing weight and enabling acquisition of digital data.In 2005, a trial was conducted to validate the reliability of the software, where already-analyzed video data was read by the software and the earlier results were compared against the analysis results of the new software system. However, the image quality of the analogue video data was rough and subtle analysis was difficult, so analysis was stopped. Accordingly, the video was digitalized for legacy migration only. To save the image data, a hard disk was used connected to the required large capacity LAN.In 2006, in order to validate the reliability of the software, an Actiwatch was also used while collecting infant data. When data of 9 infants one month postpartum was analyzed, motion detection was activated other than for direct infant movement for an average 6.0% out of an average recording time of 9.3 hours. Reliability using the Actiwatch was demonstrated by Spearman's rank correlation coefficient.During the whole development period of this system, the author participated in domestic and overseas sleep research conferences, etc. and through presentations received feedback from sleep researchers. In trials conducted with overseas joint researcher Dr. Thomas F. Anders (Department of Psychiatry and Behavioral Sciences, UC Davis, M.I.N.D. Institute), the system was refined. Further development issues that became clear are the method of automatic analysis, and the addition of voice data and vital data (breathing).
  • Development of a Care System for Multidisciplinary Approach to Cancer Care in Japan
Ministry of Education, Culture, Sports, Science and Technology:Grants-in-Aid for Scientific Research(基盤研究(A))
    Project Year: 2003 - 2006  Investigator(s):Hiroko KAMATSU
Purposes: This study was conducted to: 1) survey the current situations and existing problems in the practice of the multidisciplinary approach to cancer care; 2) develop a model for the multidisciplinary approach to cancer care that can promote the strengths of cancer treatment system and ensure the provision of patient-focused and effective care; and 3) apply the developed model to the clinical setting and evaluate whether it improve the quality of life and levels of satisfaction with healthcare among women with breast cancer. Method: As the fundamental study, a survey using the questionnaire was conducted among healthcare providers and women with breast cancer at 457 institutions authorized by the Japanese Breast Cancer Society. Based on the results, a “patient-healthcare provider information sharing" multidisciplinary approach to cancer care was developed, and an intervention study was conducted with a quasi-experimental design, in order to evaluate the effects of developed care approach. Results and Discussion: A “patient-healthcare provider information-sharing" multidisciplinary approach to cancer care was developed, with the aim of increasing the function of healthcare teams to provide human resources and opportunities to share the information of evidence-based treatment and care issues and patients' narrative information about their own lives and ways of living as well as their needs, thereby helping them fully promote their self-care abilities. Specifically, a system to provide care was developed with an emphasis placed on: 1) the assignment of resource nurses in charge of strengthening collaboration between healthcare providers and patients, 2) information systems to share patient needs and issues among the treatment team and efficient conferences that utilize them, and 3) a booklet on self-care to navigate patients. This care approach was applied to breast cancer women who were receiving neoadjuvant chemotherapy. The results suggested an increased awareness of among healthcare providers (p<0.01). Also the proportion of patients of who reported dysgeusia were significantly lower in experimental group than in control group (p<0.05).
  • Construction and evaluation of a nursing graduate school and a continued education system using e-learning
Ministry of Education, Culture, Sports, Science and Technology:Grants-in-Aid for Scientific Research(基盤研究(A))
    Project Year: 2002 - 2005  Investigator(s):常葉 恵子, Kazuhiro NAKAYAMA
The objective of this research was to explore the possibilities of utilizing e-learning in graduate schools for nurses and the continued education and training of nurses. In order to do this, a four year plan entailing the following research was undertaken to further our understanding of present conditions and needs, and to experimentally adopt and evaluate the implementation of e-learning into the above areas.1)We examined the current implementation of e-learning in various fields in universities both in and outside of Japan. We were able to indicate that the adoption of e-learning in Japan is behind that of other countries and that evaluative studies, particularly in the area of nursing are lacking.2)Registered nurses throughout Japan were subject to a questionnaire study conducted in relation to e-learning needs. Over 70% of the people surveyed were interested in participating in e-learning courses and the most popular subjects were Nursing Diagnosis, Study Techniques, Medical Health Care and Nursing Management. Factors contributing to interests in participating were concerns about the lack of communication between learners and teachers of nursing, quality of programs and the possibility of gaining credits for units completed.3)e-learning content was drawn up for nursing students and clinical registered nurses and the results evaluated. The majority of participants answered that the program assisted learning and that they would like to apply it to work later on. Replies suggesting that mutual communication of the course content is necessary were also evident among subject responses.4)In order to examine whether e-learning is in fact inferior to face-to-face lectures, randomized controlled trials were conducted. The trial consisted of four classes on the subject content of "The application of EBN in nursing clinics." The outcome produced results supporting the fact that e-learning is in no way inferior to face-to-face lectures. If anything it indicated that the rate of program completion is higher among e-learning students and that it is effective in the continued education of shift-workers. Further analytical advancement is planned in the future. These results confirm the user-friendliness of self-learning materials and the effectiveness of the application of e-learning in continued education for registered nurses. At the same time, these results also indicate the importance of the introduction of a learning management system which promotes communication between nursing students and teachers, the allocation of full-time staff and clarifying the role of educational institutions in e-learning programs.
  • Study on Acculturation and Mental Health among Nikkei Immigrants in Japan, Brazil, and U.S.A.: from a Standpoint of Medical Sociology
Ministry of Education, Culture, Sports, Science and Technology:Grants-in-Aid for Scientific Research(基盤研究(B))
    Project Year: 2000 - 2003  Investigator(s):Takashi ASAKURA
The primary purpose of this study was to examine relationships between stressors in acculturation process and health status among three major Japanese immigrants, such as Japanese Brazilians staying in Japan (JBJ), Japanese Americans in U.S.A. (JA), and Japanese Brazilians in northeastern part of Brazil (JBB), from a viewpoint of medical sociology. Due to difficulties of conducting probability sampling, our participants were not considered as the representative samples; however, we ware attempting to recruit a considerable number of participants from a relatively small population such as Japanese immigrants in each country As a result, the numbers of our valid samples were JBJ for 483; JA,553; and JBB, 201, respectively.Regarding JBJ, while having faced several difficulties to adapt Japanese society such as homesick and long working time, job stressors (i.e. over workload), as wall as racial discrimination, we found that most part of them were in the way of establishing better economic and living conditions, in terms of supporting each other within the same ethnic people. Hence, more than two thirds satisfied their Japan life. However, taking several conditions such as their poor social support form Japanese nationals, and restricted occupational status in labor market into account, they must be segregated from Japanese people in the Japan society and have poor life chances to fulfill their life goals. Our research suggested that living conditions of JBJ ware in sharp contrast to those of JBB as well as of JA, who have been living in respective host countries for more than three or four generations. Concerning JA, as succeeding generations, they were more assimilated, that is Americanized, howaver, we found mixed heritage JA as wall as the fourth generation were more stressed. Additionally, it was suggested that interment camp experiences among Nisei would influence on mental health of their succeeding generations.
  • Research on Life Stress and Social Support
Ministry of Education, Culture, Sports, Science and Technology:Grants-in-Aid for Scientific Research(総合研究(A), 基盤研究(A))
    Project Year: 1994 - 1996  Investigator(s):大島 巌, Iwao OSHIMA
Stress studies in family life, which is not only in the certain difficult conditions, but also in the general conditions, has to be discussed with the relevance to the social support approach.Firstly, in the life stress research to the general population, the hypothesis is not proved that the stress level of women was high who are housewives or working mothers during the child care period. And the distress level of the women was not correlated with their life stages or career patterns. On the other hand, the lower distress level was shown among the women whose husbands enjoyed the leisure time with their wives, and gave a lot of psychosocial supports to them.Secondly, in the nationwide follow-up survey for the families with the mentally disabled members, the family life events were observed such as decrease of the household income or disease/accident/death of household members during about five years follow-up period, and the reply of "extremely so"to the distress item of "anxiety and impatience to the future"was as high as nearly 50%. And the social resources such as supports of mental health workers and the mutual supports of family group were induced to the distressful situation, and resulted effectively.Thirdly, in the intensive follow-up survery for ten years using the interview method to the 240 families with mentally disabled members, the rate of those who took the daily care from the same family member in community at the first interview decreased to as low as 50%. And the family functioning was also weakened, although the continuous family care was given. As the change of family caretaker, 20% moved to the institution. Most of the other 80% took another family care, and those who lived independently with the aid of social care were only 15%. The new family care given mainly by siblings was very unstable. More substantial social support should be provided to the families and the mentally disabled themselves.
  • Discussion and Development on New Health and Welfare Indicators and Scales
Ministry of Education, Culture, Sports, Science and Technology:Grants-in-Aid for Scientific Research(総合研究(A))
    Project Year: 1993 - 1995  Investigator(s):Kyoichi SONODA
Regarding the study on the concepts of related to health which could be baseline of scaling, we have examined following scale terms such as, level of independence for life function, self estimation of his/her own health, degree of life satisfaction, QOL,self care, health promotion etc.in the book titled "Changing Paradigms in Health" edited by Kyoichi Sonoda & Chieko Kawata in 1995.Tsunetsugu Munakata also reviewed the scaling term related to life style, personality, QOL and so on, and proposed the role of sociological study in health related field.Wataru Koyano analyzed QOL into its components and then tried to measure the health degree from the viewpoint of level of independence for life function and reviewed the development of studies on subjective degree of life satisfaction.Based on the research study on scales of degree of life satisfaction, self esteem, health control behavior and attitude, Takashi Asakura suggested that QOL should not be measured by single scale but it should be analyzed by several components of concept.Hidehiro Sugisawa, having examined the validity of consisting concept on multi dimention scales of health locus of control developed in the United States, concluded that this scale could be applicable in Japan as these scales maintain their validity even in Japan.Iwao Ohsima studied the scale for family evaluation along with reviewing family health, aiming to contribute its care and treatment.Chieko Kawata, Yoshihiko Yamazaki and others tried to measure and evaluate complicated factors of drug abuse quantitatively and found out that the importance of comprehensive approach. Then they scaled each factors and proposed the high validity of the level of psychological support by family and the validity of the scale on intimate relationship at school and after school.Takeshi Aida has made clear the positive significant correlation among three scales of stress, degree of demand, degree of fatigue of agriculture work by examining the health related indicators for farmers.Kazuhiro Nakayama, studying community related QOL and its development, evaluated the indicators of living amenity, permanent inhabitation idea in the community, affection to local community, subjective health index, community environment and so on.
  • Lifestyle, Culture and Immigration Experiences of Korean Residents in Japan
Ministry of Education, Culture, Sports, Science and Technology:Grants-in-Aid for Scientific Research(試験研究(B))
    Project Year: 1990 - 1992  Investigator(s):Kyoichi SONODA
According to the 1991 census, Korean residents in Japan having been registered under the control of the Aliens Registration Act amount to more than 690,000. Although it is true that they are an important population segment in Japan, we have very little knowledge of the relationships among the cultural interplay between Japanese and Korean lifestyles, health needs and the overall quality of life of this major ethnic group. The main objective of this project was to conduct research on the effects of immigration and acculturation on quality of life with special emphasis on physical, mental and social well-being among Korean residents in Japan. We have completed our research under the following titles:1. Demographic and historical analysis of emigration from the Korean Peninsula into Japan.2. Differences in lifestyle and health risks among Japanese, Korean and Korean residents in Japan.3. Comparison of Health needs and hospital utilization among chronic disease patients of Koreans in Japan and Japanese.4. Differences in lifestyle and identity between Korean immigrants and their descendants in Japan.5. ACTUAL STATE OF LIFE AND HEALTH OF OLDER Koreans living alone in Japan.6. Life histories of Korean people immigrated from the Saishu Island.7. Present state of policies for foreigners in municipal corporations in Japan.
