  • A Cost Projection of Scheduled Physician Home-Visit Services in Japan: 2014 to 2064.

    This study predicts the volume and spending on scheduled physician home-visit (SPHV) services over five decades. This model-based evaluation study considered the following scenarios in Japan: (1) change in services-delivery; (2) technology-assisted services; (3) a combination of (1) and (2). The model predicted that the volume and spending on SPHV will increase as the population and working-age population decline. Scenario analysis indicated that a combined strategy could reduce the relative rate of spending to less than 2.00 in 2064, indicating that home health-care service reforms through changes in services-delivery and cost-reduction through technology-assisted services are promising in countries facing aging population.
  • Improving Patients' Medication Adherence and Outcomes in Non-Hospital Settings through eHealth: A Systematic Review of Randomized Controlled Trials.

    BACKGROUND: eHealth refers to the use of information and communication technologies (ICT) for health. It plays an increasingly important role in patients' medication management. OBJECTIVE: To assess evidence on (1) whether eHealth for patients' medication management can improve drug adherence and health outcomes in non-hospital settings and (2) which eHealth functions are commonly used and are effective in improving drug adherence. METHODS: We searched for Randomized Controlled Trials (RCTs) on PubMed, MEDLINE, CINAHL, Embase, Emcare, ProQuest, Scopus, Web of Science, ScienceDirect, and IEEE Xplore, in addition to other published sources between 2000 and 2018. We evaluated the studies against the primary outcome measures of medication adherence and multiple secondary healthcare outcome measures relating to adverse events, quality of life, patient satisfaction, and health expenditure. The quality of the studies included was assessed using the Cochrane Collaboration's Risk of Bias tool. RESULTS: Our initial search yielded 9,909 records, and 24 studies met the selection criteria. Of these, 13 indicated improvement in medication adherence at a significance level P<.1 and 2 indicated an improved quality of life at significance level P<.01. The top three functions that were employed included: mechanisms to communicate with caregivers, monitoring health features, and reminders and alerts. eHealth functions of providing information and education, and dispensing treatment and administration support tended to favor improved medication adherence outcomes (Fisher's exact test, P=.02). There were differences in the characteristics of the study population, intervention design, functionality provided, reporting adherence, and outcome measures among the included studies. Risk of Bias assessment items, including blinding of outcome assessment and method for allocation concealment were not explicitly reported in a large number of studies. CONCLUSIONS: All the studies included were designed for patient home-based care application and provided a mechanism to communicate with caregivers. A targeted study population such as older patients should be considered to maximize the public health impact on the self-management of diseases. CLINICALTRIAL: Registration number: CRD42018096627.

    AIM: To compare the tracheal intubation performance between video laryngoscopy (VL) and direct laryngoscopy (DL) in patients with cardiac arrest in the ED. METHODS: This is an analysis of the data from a prospective, multicentre study of 15 EDs in Japan. We included consecutive adult patients with cardiac arrest who underwent intubation with VL or DL from 2012 through 2016. The primary outcome was first-attempt success. The secondary outcomes were glottic visualisation assessed with Cormack grade (1 vs. 2-4) and occurrence of oesophageal intubation. To examine the between-device difference in outcome risks, we analysed the whole data and 1:1 propensity score matched data. RESULTS: Among 9694 patients who underwent intubation in the EDs, 3360 cardiac arrests (35%) were included in the analysis (90% were non-traumatic cardiac arrests). The first-attempt success rate was higher in the VL group compared to those in the DL (78% vs 70%; unadjusted OR 1.61 [95%CI 1.26-2.06] P < 0.001). This association remained significant after adjusting for six potential confounders and within-ED clustering (adjusted OR 1.33 [95%CI 1.03-1.73] P = 0.03). VL use was also associated with a better glottic visualisation (adjusted OR 3.84 [95%CI 2.81-5.26] P < 0.001) and lower rate of oesophageal intubation (adjusted OR 0.45 [95%CI 0.24-0.85] P = 0.01) compared to DL. These results were consistent in the propensity score matched analysis. CONCLUSIONS: Based on large multicentre prospective data of ED patients with cardiac arrest, the use of VL was associated with a higher first-attempt success rate compared to DL, with a better glottic visualisation and lower oesophageal intubation rate.

    In-hospital fall incidence is a critical indicator of healthcare outcome. Predictive models for fall incidents could facilitate optimal resource planning and allocation for healthcare providers. In this paper, we proposed a tensor factorisation-based framework to capture the latent features for fall incidents prediction over time. Experiments with real-world data from local hospitals in Hong Kong demonstrated that the proposed method could predict the fall incidents reasonably well (with an area under the curve score around 0.9). As compared with the baseline time series models, the proposed tensor based models were able to successfully identify high-risk locations without records of fall incidents during the past few months.

    A complex model to study the spread of influenza often requires efficient algorithms to simulate disease transmission. This article studies the internal mechanisms of existing algorithms. We compare existing algorithms to simulate disease transmission in an effort to identify impact factors and put forth rules for efficient algorithm selection. Specifically, an algorithm from the infectiousness perspective is recommended when both the transmission probabilities and the fraction of infectious individuals are small, or when the transmission probabilities are large but the fraction is either sufficiently small or sufficiently large. In contrast, an algorithm from the susceptible perspective should be adopted in the case of small transmission probabilities but a large fraction of infectious individuals, or large transmission probabilities and a moderate fraction. This investigation not only helps to guide a more-efficient simulation study of disease transmission in practice but also serves as a prerequisite for the development of more-advanced simulation models.
  • A systematic review of early modelling studies of Ebola virus disease in West Africa

    Phenomenological and mechanistic models are widely used to assist resource planning for pandemics and emerging infections. We conducted a systematic review, to compare methods and outputs of published phenomenological and mechanistic modelling studies pertaining to the 2013-2016 Ebola virus disease (EVD) epidemics in four West African countries - Sierra Leone, Liberia, Guinea and Nigeria. We searched Pubmed, Embase and Scopus databases for relevant English language publications up to December 2015. Of the 874 articles identified, 41 met our inclusion criteria. We evaluated these selected studies based on: the sources of the case data used, and modelling approaches, compartments used, population mixing assumptions, model fitting and calibration approaches, sensitivity analysis used and data bias considerations. We synthesised results of the estimated epidemiological parameters: basic reproductive number (R-0), serial interval, latent period, infectious period and case fatality rate, and examined their relationships. The median of the estimated mean R-0 values were between 1 center dot 30 and 1 center dot 84 in Sierra Leone, Liberia and Guinea. Much higher R-0 value of 9 center dot 01 was described for Nigeria. We investigated several issues with uncertainty around EVD modes of transmission, and unknown observation biases from early reported case data. We found that epidemic models offered R-0 mean estimates which are country-specific, but these estimates are not associating with the use of several key disease parameters within the plausible ranges. We find simple models generally yielded similar estimates of R-0 compared with more complex models. Models that accounted for data uncertainty issues have offered a higher case forecast compared with actual case observation. Simple model which offers transparency to public health policy makers could play a critical role for advising rapid policy decisions under an epidemic emergency.
  • Efficient heterogeneous sampling for stochastic simulation with an illustration in health care applications

    In modeling disease transmission, contacts are assumed to have different infection rates. A proper simulation must model the heterogeneity in the transmission rates. In this article, we present a computationally efficient algorithm that can be applied to a population with heterogeneous transmission rates. We conducted a simulation study to show that the algorithm is more efficient than other algorithms for sampling the disease transmission in a subset of the heterogeneous population. We use a valid stochastic model of pandemic influenza to illustrate the algorithm and to estimate the overall infection attack rates of influenza A (H1N1) in a Canadian city.
  • Statistical classification of drug incidents due to look-alike sound-alike mix-ups

    It has been recognised that medication names that look or sound similar are a cause of medication errors. This study builds statistical classifiers for identifying medication incidents due to look-alike sound-alike mix-ups. A total of 227 patient safety incident advisories related to medication were obtained from the Canadian Patient Safety Institute's Global Patient Safety Alerts system. Eight feature selection strategies based on frequent terms, frequent drug terms and constituent terms were performed. Statistical text classifiers based on logistic regression, support vector machines with linear, polynomial, radial-basis and sigmoid kernels and decision tree were trained and tested. The models developed achieved an average accuracy of above 0.8 across all the model settings. The receiver operating characteristic curves indicated the classifiers performed reasonably well. The results obtained in this study suggest that statistical text classification can be a feasible method for identifying medication incidents due to look-alike sound-alike mix-ups based on a database of advisories from Global Patient Safety Alerts.
  • Economic Evaluation of Individual School Closure Strategies: The Hong Kong 2009 H1N1 Pandemic

    Background School closures as a means of containing the spread of disease have received considerable attention from the public health community. Although they have been implemented during previous pandemics, the epidemiological and economic effects of the closure of individual schools remain unclear. Methodology This study used data from the 2009 H1N1 pandemic in Hong Kong to develop a simulation model of an influenza pandemic with a localised population structure to provide scientific justifications for and economic evaluations of individual-level school closure strategies. Findings The estimated cost of the study's baseline scenario was USD330 million. We found that the individual school closure strategies that involved all types of schools and those that used a lower threshold to trigger school closures had the best performance. The best scenario resulted in an 80% decrease in the number of cases (i. e., prevention of about 830,000 cases), and the cost per case prevented by this intervention was USD1,145; thus, the total cost was USD1.28 billion. Conclusion This study predicts the effects of individual school closure strategies on the 2009 H1N1 pandemic in Hong Kong. Further research could determine optimal strategies that combine various system-wide and district-wide school closures with individual school triggers across types of schools. The effects of different closure triggers at different phases of a pandemic should also be examined.
  • A DISCRETE-EVENT SIMULATION STUDY FOR EMERGENCY ROOM CAPACITY MANAGEMENT IN A HONG KONG HOSPITAL

    It is very common for patients to face a long journey after the first physician visit in many emergency service care models. This affects the service quality of accident and emergency departments (AEDs). In this study, we developed discrete-event simulation models to mimic the complex health service system of a 24-hour AED in Hong Kong. We assessed how changing the number of emergency department physicians or the patient journey influenced AED performance achievement, which was quantified as service achievements for patients (SAP). We observed that reducing the time spent on subsequent treatments (after the first physician visit) among semi-urgent patients was comparatively sensitive to improving the overall AED outcomes. There was an increase in mean service achievements from 69.29% to 79.30% (95% confidence intervals were 1.28% and 0.98%, respectively). The proposed model is helpful in making decisions about emergency resource planning when there is a sudden surge of emergency patients.
  • Using simulation and optimisation to characterise durations of emergency department service times with incomplete data

    Simulation models of emergency departments (EDs) are often built based on incomplete data, for example, missing arrival times or service-time durations. The difficulty in collecting reliable and complete data can subsequently lead to invalid simulation results. To tackle this problem, we propose a simulation and optimisation method to characterise the unavailable durations of service times. Since many services in an ED are sequential and dependent on each other, this paper considers these multiple process steps cooperatively. We first use lognormal distributions to characterise the key service durations. Then we propose a new meta-heuristic approach, which combines an Improved Adaptive Genetic Algorithm (AGA) and Simulated Annealing (SA), IAGASA, to search for the optimal set of service-time distribution parameters. To address the difficulties of applying IAGASA when noise is involved in the performance measures and improve the simulation efficiency, we jointly apply IAGASA and Optimal Computing Budget Allocation (OCBA) technology. OCBA minimises the total simulation cost for achieving a desired level of probability of correctly selecting the best set of distribution parameters, which improves the search efficiency significantly. The experimental results indicate that our proposed method can find accurate estimates of service-time distribution parameters within a relatively short time.
  • SCHOOL CLOSURE STRATEGIES FOR THE 2009 HONG KONG HINI INFLUENZA PANDEMIC

    Modelling of detailed community interaction dynamics increases a public health organization's ability to contain a potential disease strain at an early stage. Due to its dense population and high levels of human movements and interactions, Hong Kong has suffered from various epidemic diseases. The use of nonmedical interventions is often efficacious in containing pandemic outbreaks. In this paper, we focus on evaluating the effectiveness of various practical school-related non-medical intervention strategies to mitigate the effects of pandemic influenza under a realistic Hong Kong demographic scenario. We modelled the impact of a combination of various school closure modes, triggers, types, and lengths. The simulation results suggest that the strategy of closing all types of schools generally outperforms that of closing only a subset, especially if the closure period is substantial. We also discuss future research directions along with individual school closure and economic evaluations.

    Background: The relative contribution of long term care facilities (LTCFs) and hospitals in the transmission of methicillin-resistant Staphylococcus aureus (MRSA) is unknown. Methods: Concurrent MRSA screening and spa type analysis was performed in LTCFs and their network hospitals to estimate the rate of MRSA acquisition among residents during their stay in LTCFs and hospitals, by colonization pressure and MRSA transmission calculations. Results: In 40 LTCFs, 436 (21.6%) of 2020 residents were identified as 'MRSA-positive'. The incidence of MRSA transmission per 1000-colonization-days among the residents during their stay in LTCFs and hospitals were 309 and 113 respectively, while the colonization pressure in LTCFs and hospitals were 210 and 185 per 1000-patient-days respectively. MRSA spa type t1081 was the most commonly isolated linage in both LTCF residents (76/121, 62.8%) and hospitalized patients (51/87, 58.6%), while type t4677 was significantly associated with LTCF residents (24/121, 19.8%) compared with hospitalized patients (3/87, 3.4%) (p < 0.001). This suggested continuous transmission of MRSA t4677 among LTCF residents. Also, an inverse linear relationship between MRSA prevalence in LTCFs and the average living area per LTCF resident was observed (Pearson correlation -0.443, p = 0.004), with the odds of patients acquiring MRSA reduced by a factor of 0.90 for each 10 square feet increase in living area. Conclusions: Our data suggest that MRSA transmission was more serious in LTCFs than in hospitals. Infection control should be focused on LTCFs in order to reduce the burden of MRSA carriers in healthcare settings.

    WHO Patient Safety has put focus to increase the coherence and expressiveness of patient safety classification with the foundation of International Classification for Patient Safety (ICPS). Text classification and statistical approaches has showed to be successful to identify safety problems in the Aviation industry using incident text information. It has been challenging to comprehend the taxonomy of medical incidents in a structured manner. Independent reporting mechanisms for patient safety incidents have been established in the UK, Canada, Australia, Japan, Hong Kong etc. This research demonstrates the potential to construct statistical text classifiers to detect specific type of medical incidents using incident text data. An illustrative example for classifying look-alike sound-alike (LASA) medication incidents using structured text from 227 advisories related to medication errors from Global Patient Safety Alerts (GPSA) is shown in this poster presentation. The classifier was built using logistic regression model. ROC curve and the AUC value indicated that this is a satisfactory good model.

  • A systematic model for assessing innovation competence of Hong Kong/China manufacturing companies: A case study

    Although having moved to mainland China, Hong Kong manufacturing plus trading industry is still the largest contributor to Hong Kong GDP. However, they are facing great challenges like higher cost, lower value adding, higher pollution, and lower efficiency these years. Many of them have to be closed or moved out of the southern part of China. These companies have recognized the urgency to upgrade them by innovation. This paper reports a research that aims to analyze the innovation competence in Hong Kong manufacturing companies. The project starts with literature review and the development of a conceptual model. A structured interview guide was developed then. The model was used to evaluate the innovation competence of 7 case companies via a training and consultancy program. The literature provides a long list of critical factors or enablers for innovation (Badawy, 2011). However, they are all listed as linear, separate factors. This research proposes a multi-level model for assessing innovation competence which allows researchers to investigate strategic factors, operational factors and innovation process, separately.   • Disease surveillance by clustering based on minimal internal distance

    Disease surveillance is essential for studying disease spread. An important task in disease surveillance is identifying disease clusters, which are areas of unusually high incidence. In this paper, we formulate the disease surveillance problem as a clustering problem and review some standard techniques used for clustering problems. Inspired by techniques used in graph theory, we introduce our new method, which is based on a new statistic derived from minimal internal distance in the graph, to solve this problem. Simulated and real lung cancer data from New Mexico are analyzed according to our method, and results are compared with those of the popular spatial scan statistic. © 2012 IEEE.
  • A neural network-based approach of quantifying relative importance among various determinants toward organizational innovation

    Innovation management is the practice of managing new ideas and insights into business environment so as to increase market competitiveness. Therefore, it is vital to address the critical determinants of effective innovation management within an organizational context, hence called organizational innovation. Based on some existing frameworks of managing organizational innovation, several key determinants can be identified. Using industrial survey data, we then proposed and used a neural network-based approach to quantify the connectivity between each of these determinants and organizational innovation. First, the modeling accuracy of our proposed method was examined and benchmarked with multiple linear regression and non-linear least square fitting methods. Next, the relative importance among various key determinants toward organizational innovation can be computed using our method. We then compared the results with the qualitative judgment of field experts and professionals using the analytical hierarchy process. Based on the comparison outcomes, our proposed method is proved to be reasonably useful and practical in capturing the comparative influence among determinants toward organizational innovation.   • A methodology to generate a belief rule base for customer perception risk analysis in new product development

    New product development (NPD) is crucial for a company's success in a competitive market. Meanwhile, NPD is a process associated with great complexity and high risk. To ensure its smooth operation, risks involved in an NPD process need to be analyzed in a proper way. In this paper, a novel method is proposed to generate a belief rule base (BRB), which is the basis of the Belief Rule-Base Inference Methodology using the Evidential Reasoning (RIMER). Due to its capability in dealing with complex reasoning problems under uncertainty, RIMER is then applied to assess customer perception risk (CPR) in an NPD process. To test and validate the method proposed in this paper, a case study of an "Interactive Doll" is conducted at the end of the paper. (C) 2010 Elsevier Ltd. All rights reserved.

    The objective of public health surveillance is to systematically collect, analyze, and interpret public health data (about chronic or infectious diseases) to understand trends; detect changes in disease incidence and death rates; and plan, implement, and evaluate public health practices. Recently, studies have been conducted to develop methods and algorithms for health surveillance and disease detection. This paper attempts to review recent research on temporal and spatiotemporal surveillance methods. We have addressed specific challenges and research gaps in the relevant research. Lastly, we discuss a comparative example using a dataset of male thyroid cancer cases in New Mexico.

    Simulation emerges as an important technique in recent years for modeling complex operational dynamics in various healthcare institutions and hence providing deep insights for potential improvement. In particular, Accident and Emergency Department (A&ED) has been a place for such research as it accounts for a large proportion of the total hospital visits and admissions. To create a viable simulation for A&ED, accurate description and forecast of patient visits is the foremost step. This paper investigates several contributing factors to A&ED visits, and various time-series methods of modeling A&ED visits with different triage categories and mode of arrival. All the methods are compared in terms of goodness-of-fit and forecast accuracy. The purpose of this research is two-fold. First, this research is part of attempt to build a simulation model for A&ED of a local hospital. Second, the results may be useful for reexamine the resource allocation plan of the A&ED.

    While the challenges of the next pandemic outbreak are overwhelming, either from swine flu, other infectious disease, bioterrorism, timely detection of disease outbreaks is most important for public health surveillance and society safety and stability. In public health surveillance, the objective is to systematically collect, analyze, and interpret public health data (chronic or infectious diseases) in order to understand trends, to detect changes in disease incidence and death rates, and to plan, implement, and evaluate public health practice. Recently much research has been conducted to develop methods and algorithms for health surveillance and disease detection. This paper presents an overview and reviews the recent research methods on temporal and spatiotemporal surveillance. Specific research challenges and future research directions are discussed. A real life example is used to compare the performance of three currently used surveillance methods, scan, EWMA, and CUSUM.

    New product development (NPD) is a crucial process to keep a company being competitive. However, because of its inherent features, NPD is a process with high risk as well as high uncertainty. To ensure a smooth operation of NPD, the risk involved in the process need to be assessed and the uncertainty should also be addressed properly. Facing these two tasks, in this paper, the critical risk factors in NPD are first analyzed. Since Bayesian network is specialized in dealing With uncertainties. those risk factors are then modeled into a Bayesian network to facilitate the assessing of the risk involved in an NPD process. To generate the probabilities of different kinds of nodes in a Bayesian network. a systematic probability generation approach is proposed with emphasis on generating the conditional probabilities of the nodes with multi-parents. A case Study is also given in the paper to test and validate the critical risk factors as well as the probability generation approach. (C) 2009 Elsevier Ltd. All rights reserved.

    Purpose - Organizational innovation management (OIM) is one of the critical means to sustain competitiveness in organizational innovation in the long term. Although literature in innovation management has discussed the notions of OIM, an organization-wide OIM framework has not yet been developed and validated. This project is thus carried out to develop and validate an organization-wide OM framework. Design/methodology/approach - From an extensive review of literature, core values and concepts of an OIM conceptual system are developed. Then, OIM critical factors are identified and validated by questionnaires and interviews in the HK/PRD manufacturing industry. Findings - The OIM critical factors are validated by examining the importance of OIM critical factors and the relationship between company accomplishment in individual factors and performance in organizational innovation. Also an organization-wide OIM hierarchical framework is formulated based on the OIM factors, with an evaluation of the HK/PRD manufacturing industry. Research limitations/implications - Although the identified critical OIM factors and the developed OIM framework could cover all types of industries, this research has only been done in the electronics and electrical consumer products manufacturing industry in HK/PRD region. Practical implications - The developed organization-wide OIM hierarchical framework builds a foundation for a comprehensive study in OIM. It provides a useful reference for managers to understand critical success factors of OIM. Furthermore, this framework can be further applied for any other organization-wide OIM-based management, such as OIM improvement programmes and OIM assessment systems. Originality/value An organization-wide OIM framework is developed and validated.
